Month One - November Plant Person Interface Campaign
Plant person interface is one of the top fatal risks identified within work in construction. Following a fatality on the M3 project which involved an operator being crushed by the arm of a vacuum excavator a serious incidents review into plant related incidents was held. The review involved Highways England and the delivery partners covering four serious incidents involving plant during the past 18 months. These incidents were:
A14 – IP Hit by a dumper, major injuries, life changing injuries.
M1/A5 – IP Hit by a dumper, broken leg 8 weeks off works.
A21 – IP Hit by out of control HGV, when handbrake was not applied– Fatal.
M3 – Operative crushed by vacuum excavator/suction lifter – Fatal.
The review focused on the common factors in these incidents. The group agreed that common factors were the IP approached /entered exclusion zone around a piece of plant which was stationary / stopped working, but was idling with the engine running.
Other similarities identified were;
- Management of Stop / start / breaks in task (none of the incidents occurred during the main task activity)
- Complacency around plant.
- Perception of the risk / awareness both in the Operatives undertaking the task and those planning the task.
- Lack of appreciation of risk that 3rd parties and specialists introduce.
It has been decided to focus on the impact incidents can have on people and the simple rule of parking plant, isolating it prior to engaging in conversation or other tasks. The campaign has two videos one produced giving the recollection of a plant operator who was involved in a serious incident and a video where somebody who has been injured gives there account of the incident and its impact upon them and their family. The campaign also has a small number of posters stating that it’s as easy as P.I.E. (Park Isolate Engage) even a child could do it. The purpose is to highlight the need for correct process/procedure to be followed prior to personnel approaching/entering exclusion zones around plant

The campaign also aims to reinforce the minimum requirements and the LIFESAVER rule that delivery partners have agreed to follow in the raising the bar guidance. ;
